Dr. Susan Bailey has retired from FWAAA after 36

years of caring for patients in our practice.

Dr. Tierce and Dr. Beaty are available to assume

care of her patients to ease the transition and are

happy to continue providing the same outstanding

care that her patients have received during her time in practice here at FWAAA.

For new and existing FWAAA patients:

You can sign up for our Patient Passport in Elation, our Electronic Medical Records (EMR) System. This will allow you to request prescription refills, ask questions and access information through our EMR system. Any patients interested in enrolling will need to provide a valid email address and cell phone number, and an invitation to sign up will then be sent via email upon request. Once accepted Patient Passport can be accessed via any computer, smartphone or other device with internet access. For those who have enrolled the link to Passport is:

Before coming into the office for allergy testing, you must check our medication list to see if you have taken any of the medications prior to your testing. If you have taken any of these listed medications prior to the specific days listed in the packet, we will have to reschedule your appointment.
