Dr. Robert Rogers will be retiring on October 31
after caring for patients at FWAAA since 1984.
We are all grateful to Dr. Rogers for his years
of exemplary patient care and he will be greatly
missed. Dr. Tierce and Dr. Beaty will be available to continue managing all of his patients who require ongoing care. We all wish Dr. Rogers the very best in his retirement!

For new and existing FWAAA patients:

You can sign up for our Patient Passport in Elation, our Electronic Medical Records (EMR) System. This will allow you to request prescription refills, ask questions and access information through our EMR system. Any patients interested in enrolling will need to provide a valid email address and cell phone number, and an invitation to sign up will then be sent via email upon request. Once accepted Patient Passport can be accessed via any computer, smartphone or other device with internet access. For those who have enrolled the link to Passport is:

If you have taken any of these listed medications prior to the specific days listed in the packet, we will have to reschedule your appointment.
