New Patient Appointments- Allergy Testing

 

If you are planning on having an allergy test performed during your appointment, please refer to the list of medications linked below that must be avoided prior to allergy testing. 

 

If you have taken any of these listed medications prior to the specific number of days listed, we may have to reschedule your appointment.  Please call our office at (817)315-2550 if you have any questions.

If your paperwork is not completely filled out, we may have to reschedule your appointment for a later date.

 

Copies of all paperwork can also be mailed to you or picked up at the receptionist desk in our office.

 

New Patient Checklist:

 

1) Your current insurance card

 

2) Physician referral forms, if required by insurance

 

3) List of current medications, including over-the-counter medications, with dosage and frequency

 

4) Completed New Patient Packet

 

 

Patient Portal:

Please call our office at (817)315-2550 so that we can help you access our online Patient Portal.  This will allow you to access your private health information securely as well as to communicate securely with your physician and the office staff regarding your visit summaries, medications, lab results and more.

​

Please provide your current cell phone number and email address.  An invitation will be sent to your email inbox, and your cell phone number will be needed for account authentication to complete your setup for the Portal.

​
